Obtain Forms

Purchase application forms from the college admissions office or download them online.

Complete Application

Fill in all sections of the form accurately and attach all required documents.

Submit Application

Submit your completed application form in person or by post before the deadline.

Entrance Assessment

Shortlisted candidates will be invited for a written assessment and interview.

Admission Letter

Successful applicants receive an official admission letter with reporting instructions.

Completed application form
Original and copies of academic certificates (BECE/WASSCE/SSSCE)
Birth certificate or passport
Two (2) passport-sized photographs
Medical fitness certificate from a licensed physician
Police clearance certificate
Letter of recommendation (for Post Basic Midwife applicants)
Valid nursing licence (for Post Basic Midwife applicants)
Application fee payment receipt
